What Causes Neck Pain Extending Into the Arm?

When there is surgery upon a neck or the neck pain was caused in a Motor Vehicle Accident, there is a much higher chance that chronic pain is the result. Even worse is when the automatic nervous system (Autonomic Nervous System) becomes activated. This can occur due to the inflammation and pain resulting from surgery. Pain following a neck surgery or from an automobile accident will cause insufficient circulation to the bone underlying the discs of the neck and where ligaments attach to the bones of the neck.

Neck pain is a common problem with up to 70% of us having neck pain at some point in our lives. In many cases, neck pain occurs when there is insufficient circulation to the bone underlying the discs of the neck and where ligaments attach to the bones of the neck. Discs serves as a cushion in joints between bones. This allows for the absorption of shock and allows motion of one bone against another. This can be a result of “wear and tear” or injury.

The discs have no circulation of their own. They use the circulation of the underlying bone to provide oxygen and remove waste products. However, if the bone does not have sufficient circulation, the portions of the bone which require more oxygen, such as beneath cartilage, begin to die; this creates dead spots in the bone beneath the discs, called subchondral cysts. When cold weather approaches and the barometric pressures fall, the subchondral cysts expand against the injured portion of the discs and causes pain. Unfortunately, pain causes a further decrease in circulation to the bone making the neck pain even worse.

The same thing happens in the areas where ligaments attach to the bones of the neck. In these areas there is an increased need for oxygen and the bone dies creating subchondral cysts. Since the ligaments and muscles are now attaching to areas of weakened bone, they can pull away from the underlying bone. As the ligaments and muscles pull away the covering of the bone, new bone will fill in this area making a projection of bone. The bone projection can push on nerves, which go into the arms. This can make the arm tingle with a “funny bone” sensation.

As the discs begin to degenerate the joint space narrow and the joints become painful and stiff as there is an increase in bone to bone contact.

The nerve chemical [Calcitonin gene-related Peptide (CGRP)] which increases circulation to the bone is the same chemical which controls circulation to the fingers and toes. This is why many patients with neck pain also have decreased circulation causing cold toes and many times cold fingers.

The neck pain is even worse when there is a second nerve chemical also insufficient [Vasoactive Intestinal Polypeptide (VIP)]. VIP decreases inflammation throughout the body. In this case, inflammation caused by the degeneration of the bones and discs is even worse due to a lack of VIP. The pain caused by the inflammation decreases the circulation and makes the problem worse. In many patients, the lack of VIP also causes airborne and food allergies, as well as, digestive problems. Not only does neck pain interfere with quality sleep, but since VIP is also necessary to create melatonin, many of these patients also have insomnia.

Pain causes further decrease in circulation because it destroys CGRP. CGRP not only increases circulation to the bones but also increases circulation to the nerves that created VIP. Therefore, if CGRP is decreased, then VIP is also decreased.

Another common source of pain associated with neck pain is osteoporosis, which is a progressive decrease in bone density and, therefore, bone strength. A lack of sufficient CGRP and VIP can cause osteoporosis. Not only does CGRP increase circulation to the bone but also CGRP and VIP cause increased bone creation while decreasing bone degradation. One of the most common treatments of neck pain is physical therapy. Physical therapy should never cause pain when performed because it will then diminish the circulation at the cellular level (CGRP).

Many of the other treatments for neck pain can cause the neck pain to actually increase and/or cause other problems. NSAIDs are well known for causing stomach pain and stomach ulcers. Injections cause a response from the Autonomic Nervous System (automatic nervous system). Chronic pain (pain lasting longer than 6 weeks) involves the Autonomic Nervous System (ANS).
